What Ambetter typically covers

Ambetter marketplace plans and outpatient addiction treatment in New Hampshire

Ambetter is the marketplace brand of NH Healthy Families, part of Centene. If you bought your own coverage on HealthCare.gov rather than through an employer, there is a good chance it is an Ambetter bronze, silver or gold plan. Every marketplace plan must cover substance use treatment as an essential health benefit, and federal parity law requires it to be covered on the same terms as medical care. The differences between plans are in the deductible, the copays and the authorization rules, not in whether treatment is covered.

At our Londonderry center, Ambetter members typically use these levels of care:

  • Partial Hospitalization (PHP). Many Ambetter plans cover PHP when a clinical assessment supports it, with authorization approved before the first day.
  • Intensive Outpatient (IOP), day or evening. Many plans cover IOP, usually with an authorization step up front.
  • Outpatient (OP). Weekly therapy is billed like other specialist visits, with a copay or coinsurance.
  • Medication-Assisted Treatment (MAT). The visits are covered like other outpatient care. The medication runs through the pharmacy benefit and Ambetter's formulary.

Coverage depends on your plan. A bronze plan with a high deductible can mean paying the full contracted rate until the deductible is met, while a silver plan with cost-sharing reductions can mean a small copay from the first visit. We verify your benefits in one call and tell you where your plan stands before anything is scheduled.

Levels of care

Every level is outpatient. You sleep at home and keep your life around treatment. Detox, when it is needed first, is arranged by referral to a partner facility.

Prior authorization and copays

How the paperwork usually works with Ambetter

Admissions and our clinical team handle every step of this. Here is what it looks like from your side.

  1. 01

Authorization is the norm: Ambetter plans generally ask for prior authorization for partial hospitalization and intensive outpatient care, reviewed against the ASAM Criteria. Our clinicians send the assessment and treatment plan in with the request.

  2. 02

Outpatient usually needs none: Standard outpatient therapy visits generally need no approval. If your plan is an HMO, it may want your primary care provider listed as the referring doctor, and we help with that.

  3. 03

Deductible first on most plans: Bronze and many silver plans apply the deductible before the plan pays for outpatient treatment. Silver plans with cost-sharing reductions, and gold plans, usually charge a copay per visit from the start. For 2026, marketplace out-of-pocket costs cannot exceed $10,600 for one person or $21,200 for a family.

  4. 04

Keep the premium paid: Marketplace coverage ends if premiums lapse, and a grace period can leave claims pending. If money is tight, tell us. Admissions can help you check whether you still have coverage before you start.

Ambetter Health is listed among the plans accepted at our New Hampshire program, at 1D Commons Dr, Units 21 & 22, Londonderry, NH 03053, off Route 102 at I-93 Exit 4, about 15 minutes from Manchester and 20 from Nashua and Salem. Ambetter from NH Healthy Families is a New Hampshire plan, and networks are plan-specific, so during the benefits check we confirm your exact plan against our contract.

If you have an Ambetter plan from another state, call first and admissions will confirm which office, if either, your plan pays for.

One thing we say plainly: we are not in network with Medicare at this time. If Medicare is your primary coverage, call anyway. Admissions can point you to options, and FindTreatment.gov, the federal treatment locator, lists programs that accept it.

Ambetter questions

Ambetter and rehab in New Hampshire: common questions

Short answers here, and a real one for your plan when you call (603) 769-8981.

Does Ambetter cover IOP in New Hampshire?
Many Ambetter plans cover intensive outpatient treatment when a clinical assessment supports it, usually with authorization first. What you pay per session depends on your plan's deductible and copay. Call (603) 769-8981 and we will check before you start.
Does Ambetter cover PHP, partial hospitalization?
Partial hospitalization is a covered level of care on most Ambetter plans and needs approval before day one. Our clinicians send the assessment and treatment plan and handle the reviews during treatment.
Is Ambetter the same as NH Healthy Families?
Same company, different products. NH Healthy Families is the Medicaid plan and Ambetter is the marketplace plan sold on HealthCare.gov. If your card says NH Healthy Families without Ambetter on it, read our NH Medicaid page. Both are accepted at our Londonderry program.
Does Ambetter cover Suboxone or Vivitrol?
The visits are covered like other outpatient care. The medication goes through your pharmacy benefit, which may prefer one product over another. We check both sides in the same call.
Do you take Medicare?
We are not in network with Medicare at this time. If Medicare is your primary insurance, admissions can point you to options, and FindTreatment.gov lists programs that accept it.
