An intensive outpatient program built around a workday
Evening IOP is our Intensive Outpatient Program on an after-work schedule. Groups meet Monday through Thursday from 6 to 9 pm at our Londonderry center, four evenings a week, twelve hours of structured treatment in total. You go to work, school or your kids' pickup during the day, drive to Londonderry for the evening session, and sleep in your own bed. The program is run by the same licensed clinicians who run our daytime IOP, with the same care plan, the same group curriculum and the same medical oversight from our Medical Director, Dr. Richard Marasa, MD.
Each evening combines group therapy with skills work: relapse prevention, managing cravings and stress, repairing relationships, and understanding how substance use and mental health feed each other. Individual counseling sessions are scheduled around the group nights, and medication-assisted treatment is available when it fits your plan. Nothing about the evening track is a lighter version of treatment. It is the full program at a time of day that working adults can keep.

Evening IOP tends to be the right level of care when you need real structure but cannot step away from your life to get it. It is a good fit if you:
Work days, run a business or attend classes and cannot take weeks off.
Have a stable, safe place to live and people around you who support your recovery.
Are stepping down from Day Treatment, a residential program or detox and want to keep the momentum going.
Have tried to stop on your own or with weekly therapy and it has not held.
Need medication support for opioid or alcohol use disorder alongside counseling.
Are new to treatment and want a program you can start this week without rearranging everything.
It is not the right starting point for everyone. If withdrawal could be medically risky, if you need round-the-clock support, or if home is not a safe place right now, a licensed clinician will say so at your assessment and help you plan a safer first step. Medical detox is not provided on site. When it is needed first, we arrange it with a partner facility and hold your place in the evening program for when it is complete.
Evening versus daytime
How Evening IOP differs from daytime IOP
Both tracks are the same Intensive Outpatient Program. What changes is the clock, and a few things that follow from it.
Daytime IOP
Sessions during business hours, up to five days a week. Suits people who are not working right now, who are stepping down from Day Treatment, or who want the most contact time and the shortest overall program.
Four evenings a week, Monday through Thursday, 6 to 9 pm. Suits people who work or study during the day. Friday through Sunday are yours, which is why the evening track leans hard on weekend planning and a written relapse-prevention plan.
The group in the evening track is made up of people with jobs, classes and families, so the conversation tends to center on the situations they walk into the next morning: the coworker who drinks at lunch, the commute past the old bar, the family dinner. Clients who start in the daytime program sometimes move to evenings when they return to work, and the reverse happens too. Your clinician adjusts the schedule with you rather than making you choose once.
What a week looks like
A week in Evening IOP at our Londonderry center
Every plan is personal, and the mix of groups shifts as you progress, but a typical week runs like this:

Monday, 6 to 9 pm: Check-in and process group. How the weekend went, what came up, what you used your plan for. Then a skills block on cravings and triggers.

Tuesday, 6 to 9 pm: Psychoeducation and group therapy. How substance use changes the brain, what recovery does to reverse it, and how mental health symptoms fit into the picture.

Wednesday, 6 to 9 pm: Relationships and family. Communication, boundaries and repairing trust, with family sessions scheduled for the people close to you when that helps.

Thursday, 6 to 9 pm: Relapse prevention and weekend planning. You leave with a written plan for Friday through Sunday, and you know exactly who to call if it wobbles.

Around the groups: An individual session with your counselor each week, medication visits with our medical team if MAT is part of your plan, and drug and alcohol screening as your plan requires.
How long you stay in Evening IOP depends on your assessment and your progress, not a fixed calendar. Most people step down to our Outpatient Program and then to aftercare, which is planned before the evening program ends rather than after. Getting here after work
Londonderry is a short drive from most of southern New Hampshire
Our center is at 1D Commons Dr, Units 21 & 22, Londonderry, NH 03053, just off Route 102 at I-93 Exit 4, with free on-site parking. Leaving work at 5 or 5:30 gets most people here in time for a 6 pm start: about 15 minutes from Manchester, 20 from Nashua or Salem, 10 from Derry, 12 from Windham, 20 from Hudson and about 35 from Concord. If you commute into Massachusetts, I-93 north brings you straight to Exit 4.
Most health plans cover intensive outpatient treatment, and the evening schedule does not change the benefit. Our Londonderry program works with Ambetter Health, AmeriHealth, Blue Cross Blue Shield, Harvard Pilgrim, NH Healthy Families, Point32Health, TRICARE, Tufts Health Plan, VA Community Care and WellSense Health Plan, and with New Hampshire Medicaid through Granite Advantage plans. What are the Evening IOP hours?
