We do not run a detox. Here is what we do instead.

Clear Steps Recovery has no detox beds, no residential rooms and no overnight care at our Needham, MA center or our Londonderry, NH center. Every level of care we provide is outpatient: Day Treatment on weekdays, Evening Treatment on Monday through Thursday evenings, a weekly outpatient program, medication-assisted treatment and a mental health track. What we do provide, and what this page is about, is the referral. When the clinician who assesses you decides that withdrawal needs medical supervision before outpatient treatment can safely start, we arrange that detox stay at a partner facility and we plan your step into our program for the day it ends.
That distinction matters because searching for detox near Boston brings up a lot of programs that want to keep you for weeks. Our interest is different. We want you medically stable and then back home, in treatment during the day or in the evening, sleeping in your own bed. Detox is the first step in that plan, not the plan itself.

Who needs detox first

When medical detox has to come before outpatient care

Not everyone who starts treatment needs a detox stay. Many adults with alcohol or drug problems begin directly in Day Treatment or Evening Treatment. A clinician makes that call at your assessment, and these are the situations where a referral to medical detox usually comes first:
  • Heavy daily drinking, especially with a history of withdrawal seizures, blackouts or delirium tremens
  • Daily benzodiazepine use (Xanax, Klonopin, Valium, Ativan), because stopping suddenly can be medically dangerous
  • Opioid dependence where a medically supervised start on buprenorphine or another medication is the safest first step
  • A medical condition, pregnancy or a previous complicated withdrawal that needs monitoring
  • No safe, sober place to be during the first days off the substance

How the referral works

How we arrange a detox bed at a partner facility

We work with licensed medical detox facilities in Massachusetts, the ones we have sent people to before and whose discharge planners know how to reach us. When the assessment points to detox, admissions contacts the partner facility, shares what they need for admission with your consent, and confirms the bed and the arrival time with you or your family. The detox facility provides the medical care during withdrawal: monitoring, comfort medications and, for alcohol and benzodiazepines, the medications that prevent seizures. Clear Steps does not provide any of that. We provide the coordination on both ends.
While you are in detox, your place with us is already held. We speak with the partner facility about your expected discharge date and schedule your first day in Day Treatment or Evening Treatment to match it, so you leave detox with an appointment, not a phone number. If the facility you prefer is not one we have worked with, tell us. We will still coordinate the handoff, because the point is that nobody goes home from detox with nothing scheduled.

What a partner detox stay usually looks like

Most medically supervised detox stays run several days to about a week, depending on the substance, how long you have used it and your overall health. The partner facility sets the length, not us. Insurance usually covers all or part of a medically necessary detox stay, and we verify both the detox benefit and your outpatient benefit before anything is scheduled, so you know what to expect between the two.

The step down into our Needham program

Coming out of detox you start at the level a clinician recommends: Day Treatment, which your insurer may call PHP, on weekdays with no overnight stays, or Evening Treatment on Monday through Thursday evenings if you are going back to work right away. Medication-assisted treatment started in detox continues with our medical team without a gap. From there you step down to the weekly outpatient program and aftercare as you stabilize, all with the same clinicians at 392 Chestnut Street, Suite 200 in Needham.

Does Clear Steps Recovery provide detox in Boston or Needham?
No. We are an outpatient program and we do not provide medical detox on site at our Needham, MA center or anywhere else. When a clinician decides that withdrawal needs medical supervision first, we arrange a bed at a partner detox facility in Massachusetts and hold your place in Day Treatment or Evening Treatment for the day you are discharged.
Does insurance cover a detox referral and the outpatient program after it?
Most private plans and MassHealth cover all or part of a medically necessary detox stay, and outpatient treatment is covered under federal parity law on the same terms as other medical care. We verify both benefits with your carrier at no cost before anything is scheduled. Plan pages for our Needham center: MassHealth, Harvard Pilgrim and Tufts, Mass General Brigham Health Plan and UnitedHealthcare and Optum. We are not in network with Medicare at this time.
